Application of a hierarchy of risk controls is a methodology used in safety management to minimize or eliminate exposure to hazards and to eliminate or reduce risk of injury to as low as reasonably practicable. It is a widely accepted methodology with applications ranging from highly hazardous processes with potential for catastrophic consequences to life, property and the environment to preventing injuries when using portable hand tools. This paper provides guidance on the practical application of the risk control measures in both the design of workplace facilities and in task planning for construction, operation, maintenance and demolition work activities. Furthermore, many health problems occur later, not immediately on exposure, and may be cumulative. Consequently, it is difficult to conclusively identify the cause. Workers may lack knowledge of long-term consequences, and thus not use protective systems effectively.

Compounding this is the lack of instruments and methodologies to measure exposure to harm. Historically, the existing risk methodologies for calculating safety risk are based on the construct of consequence and likelihood. However, this may not be appropriate for health, especially for the long-term harm, as both the consequence and likelihood may be indeterminate. At present, quantitative assessment of electrical safety considering the operation conditions of large-scale electric vehicle charging stations (EVCSs) has still remained a challenge. Driven by the hierarchy of hazard control mechanisms, this paper proposes a holistic approach to evaluate the electrical safety of the large-scale EVCSs when coupled to renewable power generation. Our approach mainly focuses on several topics on the operational safety of EVCS primarily concerning: (1) the facility degradation which could potentially result in a compromised EVSE reliability performance and EVCS protection failure; (2) the cyber-attack challenges when the smart charging and the communication between EVCSs and electric utilities are enabled; (3) the potential mismatch between the renewable output and EVCS demand, which could trigger the system stability challenges during normal operation and inability to supply the critical EV loads during outages. The proposed framework will provide informative guidelines to the EVCS operators for continuous monitoring and effective management of the day-to-day EVCSs operation.

Landis Floyd For the first time, the 2015 revision to National Fire Protection Association (NFPA) 70E made reference to a more than 50-year-old safety management methodology, with the reference to the hierarchy of risk controls in Section 110.1(G). This paper discusses the difference between the risk control measure “elimination” as defined in occupational health and safety management systems standards such as ANSI Z10, and the “electrically safe work condition” as defined in NFPA 70E. Establishing an electrically safe work condition is more accurately described as a series of administrative controls, including planning, drawings and documentation, lockout tag out, safe work practices, and personal protective equipment. Each of these administrative controls has some degree of residual risk. An electrically safe work condition is not permanent, and has both time and physical boundaries that must be adhered to in order to avoid exposure to dangerous energy. Elimination, as defined in occupational health and safety management systems standards, is absolute, permanent, and with no residual risks for the life of the installation. Injury numbers and rates were used to compare and trend electrical injury experience for various groups and categories.

This information allowed identification of at-risk groups that could most benefit from effective electrical safety interventions. The data presented in this paper are derived from the U.S. Labor Department's Bureau of Labor Statistics' Census of Fatal Occupational Injuries, Survey of Occupational Illnesses and Injuries, and Current Population Survey. Between 1992 and 2002, 3378 workers died from on-the-job electrical injuries. Electricity remained the sixth leading cause of injury-related occupational death. From 1999 to 2002, 4.7 of all occupational deaths were caused by electricity, down from 5.2 in the 1992-1998 time period. The cause of death was listed as electrocution in 99.1 of fatal cases. Contact with overhead power lines was involved in 42 of all on-the-job electrical deaths. The construction industry accounted for 47 of all electrical deaths between 1992 and 2002 but showed overall improvement from 1995 to 2002 by reducing its electrical fatality rate from 2.2 to 1.5 per 100 000 workers. In addition, 46 598 workers were nonfatally injured by electricity. Contact with electric current of machine, tool, appliance, or light fixture and contact with wiring, transformers, or other electrical components accounted for 36 and 34 of nonfatal electrical injuries, respectively. Contact with underground buried power lines was involved with 1 of fatal injuries and 2 of nonfatal injuries. Some of my most memorable work experiences happened in the four short months I worked there. I remember not receiving any electrical safety training. Electrical work was done live without a second thought. It’s hard to believe now, looking back, how nonchalant we were about electricity. That nonchalance undoubtedly caused by a lack of awareness. We were electrical maintenance workers, and management assumed we knew how not to get killed. I remember when an older electrician named Bill gave me the arc flash training. It wasn’t called arc flash training because none of us had ever heard that phrase before. We didn’t know what happened when panels blew up or that it had a name. The information was given as somber advice while we were standing in front of a large disconnect.

Bill said, “When you open or close one of these big disconnects don’t stand in front of it. Stand off to the side, use your left hand, turn your head and it might not hurt to duck a little.” I asked why and he said, “sometimes these things blow up.” That was good advice then and is still relevant today. The problem was that in 1984 that advice was the entire arc flash class. That was all we knew. Several years later I was repairing a variable frequency drive when part of it exploded when I closed the breaker. I was standing off to the side and was not injured. Thanks, Bill. Stand off to the side, use your left hand, turn your head and it might not hurt to duck a little.” Thirty years ago we wrapped electrical tape around screwdrivers and called them insulated. We thought the plastic dipped handles on our Klein lineman’s pliers made them safe. Channel locks were fuse pullers. We wore polyester shirts and gold chains, and our only PPE was a hard hat, steel-toed shoes and safety glasses. A lot has changed in regards to electrical safety and electrical safety training in the decades since. Arc-rated clothing has replaced our polyester shirts, voltage-rated insulated tools have replaced electrical tape, and most importantly, awareness of electrical hazards has replaced our ignorance. We can no longer work as we did back then, and we have to receive training on electrical hazards. In the ten years, I worked in factories, my employer never gave me formal electrical safety training. The first electrical safety class I was a part of I was the instructor. In this Practical Guide To Arc Flash and NFPA 70E, I hope to simplify parts of the standard. This guide is not going to answer every question you have, and it doesn’t address every article in 70E.
